Billing/Finance Policy
Payment Expectations
Payment for services is expected at the time of your visit unless prior arrangements have been made. We accept cash, checks, and major credit cards. Co-pays, co-insurance, and deductibles are due at the time of service.
Insurance
We participate in most major insurance plans. As a courtesy, we will file your insurance claims for you. However, it is your responsibility to understand your insurance benefits and to ensure that our providers are within your network.
Statements
If you receive a statement, payment is due upon receipt. If you are unable to pay the full amount, please contact our billing department to discuss payment options.
Financial Hardship
If you are experiencing financial difficulties, please contact our office. We may be able to offer payment plans or discuss other financial assistance options.
Outstanding Balances
Patients with outstanding balances must make payment arrangements prior to scheduling appointments.
Appointment Policy
Scheduling
Appointments can be scheduled by phone or through our online portal. Please arrive 15 minutes before your scheduled time to complete any necessary paperwork.
Cancellations/No-Shows
If you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance. After two no-shows without prior notice, we will not offer another appointment.
Late Arrivals
If you arrive more than 30 minutes late for your appointment, we will do our best to accommodate you. However, adjustments may need to be made to ensure other patients are seen on time.
Emergency Appointments
We understand that emergencies happen. If you need to be seen urgently, please contact our office, and we will do our best to accommodate you.
Prescription Refill Policy
Refill Requests
Please allow 48 hours for prescription refill requests to be processed. You can request refills through your pharmacy or our patient portal.
Controlled Substances
Refill requests for controlled substances will be closely monitored. Patients must have regular follow-up appointments to continue receiving these medications.
Prior Authorization
If your insurance requires prior authorization for a medication, we will submit the necessary paperwork. Please note that this process can take several days.
Patient Visits
To receive a prescription refill, patients must have been seen within the past calendar year.
Lost Prescriptions
If you lose a prescription for a controlled substance, it may not be refilled until your next appointment. Please keep your prescriptions in a safe place.
Privacy Policy

Release of Information
We will not release your medical information without your written consent, except as required by law.
Access to Records
